Applicant Requirements
To ensure Midtown Rest Haven Boarding Home provides a safe, respectful, and supportive environment for all residents, applicants must meet the following general admission requirements.
Basic Eligibility
Must be 18 years of age or older.
Must complete the resident application in full.
Must provide a valid government-issued photo ID.
Must participate in the admission interview and assessment process.
Must agree to follow all boarding home policies, procedures, and house rules.
Required Documentation
Applicants may be asked to provide:
Government-issued identification
Social Security card or proof of identity
Proof of income or benefits (such as SSI, SSDI, VA benefits, pension, or employment income)
Health insurance information, if applicable
Emergency contact information
Physician and pharmacy information
Guardian or case manager information, if applicable
Health & Safety
Must be able to reside safely in a boarding home setting.
Must disclose medical conditions, allergies, medications, and emergency medical information needed for safe care coordination.
Must comply with medication and safety policies while residing at the facility.
Financial Responsibility
Applicants must demonstrate the ability to meet monthly occupancy charges or have an approved source of payment.
All required admission documents and agreements must be completed before move-in.
Community Expectations
Residents are expected to:
Treat fellow residents, staff, and visitors with dignity and respect.
Maintain appropriate personal conduct.
Follow all house rules and safety policies.
Help maintain a clean, peaceful, and respectful living environment.
Refrain from violence, threats, illegal drug activity, theft, or other behavior that endangers others or disrupts the community.
Admission Decisions
Admission is based on the applicant’s individual circumstances, the availability of accommodations, and whether Midtown Rest Haven Boarding Home can safely and appropriately meet the applicant’s housing and support needs while maintaining the well-being of the resident community.
